Healthcare Practices: Payroll Features Every Business Needs (2026)

Healthcare practices need payroll configured for shift differentials, on-call pay and credentialing-based pay tiers, pay codes that general-purpose software leaves to the customer to build. BEG runs healthcare practice payroll for $45 per employee per month inside the payroll software you already use, all-inclusive, live in 3 to 5 business days.

Pay codes a healthcare practice cannot run without

Healthcare practices pay physicians and nurses on different bases, and the payroll system has to hold every one of them as its own pay code with the right tax and overtime treatment: shift differentials, on-call pay, credentialing-based pay tiers and multi-location clinic payroll. A generic setup collapses these into hourly and salary, and the errors show up in the regular rate, in year-end forms and in the first wage claim.

Hospitals and residential care establishments may compute overtime over a fixed 14 consecutive day work period instead of the 40 hour workweek, paying time and one half for hours over 8 in a day and over 80 in the period. The employer must have a prior agreement or understanding with the affected employees before the work is performed, and cannot use both systems for the same employee.

Shift differentials and non-discretionary bonuses such as attendance, retention and supplementary shift bonuses must be added into the regular rate before the overtime premium is computed. Paying time and one half of the base hourly rate alone is a violation when differentials are paid.

Tax handling specific to healthcare practices

An employer that averaged at least 50 full-time employees, counting full-time equivalents, in the prior calendar year is an applicable large employer and must offer affordable minimum essential coverage to full-time employees and their dependents or face an employer shared responsibility payment. A full-time employee is one with at least 30 hours of service a week or 130 hours a month; the base payments are $2,000 and $3,000 per employee per year, indexed, and were $2,970 and $4,460 for 2024.

What is configured before the first run

At implementation every pay structure above is built as a code with its own taxability, its own overtime treatment and its own W-2 mapping, and the exemption status of physicians and nurses is documented against the rule that supports it. That is the difference between software with the features and a payroll that uses them correctly.

Frequently Asked Questions

Can you handle shift differentials for nurses?

Yes. Shift differentials are set up as pay rate modifiers. Night shift, weekend, and on-call differentials apply automatically based on hours worked, producing the correct blended rate and overtime calculation.

How do you handle ACA reporting?

For practices with 50 or more full-time equivalent employees, we handle Form 1094-C and 1095-C preparation and filing. We track hours to determine ACA full-time status and flag coverage gaps.

Can you process payroll for multiple clinic locations?

Yes. Multi-location practices run on a single payroll system with consolidated reporting and per-location cost center codes.

How do you handle credentialing-based pay tiers?

Credentialing-based tiers are configured as pay grade structures. When a provider advances, the pay tier updates on the date you specify.

What does BEG charge for healthcare payroll?

Healthcare payroll runs at $45 per employee per month, all-inclusive. For a 25-person practice, that is $1,125 per month.

What does managed payroll cost for a healthcare practice?

$45 per employee per month, all-inclusive, inside the payroll software you already use, with no monthly minimum. Shift differentials, on-call pay and credentialing-based pay tiers are configured at setup, not billed as add-ons.
